چکیده
Clinical trials evaluating negative pressure wound therapy (NPWT) as a treatment for ulcer (PU) have been inconclusive. The objective of this research was to evaluate the effect NPWT on length stay (LOS), charges, and mortality PU patients. data collected over 4-year period from large medical institution in OMNY Health System Database were used identify adult Patients excluded if also diagnosed with diabetic, chronic skin non-pressure, or varicose ulcers. Current Procedural Terminology codes divide patients into comparison cohorts. Crude estimates (percent 95% confidence interval [95% CI]) LOS charges (median, quartiles 1 3 [Q1, Q3]) generated each outcome. Propensity score methods account underlying differences between After exclusions, 749 divided (N=135) (N=614) slightly greater cohort (28.9%; CI: 21.2%-36.5%) than (22.2%; 18.9%-25.4%). incurred notably higher (median [Q1-Q3]: $65,197 [$32,232-$143,607] vs. $13,850 [$3,056-$35,929]) 11 days [7, 22] 5 [3, 9]) application propensity methods, adjusted relative risk 1.20 (95% 0.88-1.65), median 5.8 (Q1-Q3: 3.9-9.2) $30,900 $15,434-$58,868), respectively. No notable positive effects outcomes hospitalizations observed even after adjusting differences. Residual confounding by indication could be possible explanation associations outcomes. Further is needed examine longer term such readmission rates drivers cost difference.

Home health agencies, challenged to demonstrate quality while containing costs, are motivated to find best practices for managing patient and wound care. The effects of different wound therapies on frequency of hospitalization and emergent care, two prominent quality measures, have not been studied. A retrospective study was conducted to determine the prevalence of Stage III and Stage IV pressu...

Venous ulcers occurs in 1% of adult population and is associated with chronic disability, diminished quality of life and high health care costs. Treatment is often slow, difficult and challenging. Poor prognostic factors include large ulcer size and prolonged duration. Moreover the recurrence rates may be as high as 70%. We present case report of 58 years old woman with very large bilateral ven...

Successful treatment of peritonitis with negative pressure wound therapy has been frequently reported. Long term open abdomen treatment in complex cases such as in the treatment of peritonitis, often leads to an extensive abdominal wall defects. It may be challenging to close such a defect. Both primary sutures and synthetic meshes are strongly discouraged in surgical treatments of such cases. ...
